CONTRACTOR LICENSE REGISTRATION / RENEWAL FORM
Please be advised that all contractor licenses expire one year from the date of issuance. It is the contractor’s responsibility to renew the license before its expiration and to provide updated insurance forms. If the license expires, a $40 late fee will be assessed. To renew the license before expiration, please submit the renewal form, new insurance documentation, and a $20 renewal fee. Payments can be made by mailing a check to 11307 Business Park Circle, Firestone, CO 80504, or by paying in person with a credit card.
Additionally, all contractors must maintain liability insurance coverage of at least $1,000,000, specifying workmans compensation coverage. The Certificate of Insurance must name the District as additional insured. A duplicate copy of the renewed license with the new date of issuance will be issued upon receipt of the required documents.
I acknowledge and agree to be responsible for all work performed by my company in the District and to adhere to the St. Vrain Sanitation District’s Rules and Regulations. I understand that SVSD conducts inspections only during normal business hours. If an inspection is required outside of these hours, I will be responsible for a minimum payment of $130 prior to the inspection being conducted (see section 3 of Appendix A in District Rules and Regulations).
I have reviewed and understand St. Vrain Sanitation District’s Construction Standards, General Notes, and Details. I declare, under penalty of perjury, that the information provided is true and correct to the best of my knowledge.
Failure to comply with St. Vrain Sanitation District’s Rules and Regulations may result in the termination of the license and potential fines.
